Does anyone have a copy of the Request Continuation form now required to be submitted in response to a Notice of Termination of Child Support? I know it can be requested from probation. The NJchildsupport.org website has a nifty infographic and video that references the form but of course the form is not available on their website or anywhere else for that matter. Also, of note, a transcript or tuition invoice is not sufficient to prove enrollment the only acceptable documents are either a letter from the school on letterhead OR an enrollment verification form (provided by the school or third-party vendor). If you can't get proof of enrollment on the school's letterhead, you can go to this website they charge a $2.50 fee:
