Request for Alimony Pendente Lite in Philadelphia

I have a client in a divorce litigation in Philadephia who needs to file a request for alimony pendente lite. Does anyone have a sample I could review?

Delaware County has a local rule that it should be filed as an "application" with the domestic relations section and has a simple form to fill out, but I can't find anything similar for Philadelphia. Has anyone done this before? Thank you.
