    Good morning.  Does anyone have guidance on how to register an unattended birth from nearly two years ago?  The mother (prospective client) became pregnant during the pandemic, never had any prenatal care, and gave birth at home in August 2020.  The child has never had any pediatric care or immunizations because she could not prove the child is hers without a birth certificate.  The mother is looking to properly register the birth.

    Stephanie,

    You will need to go to the NJ Dept of Health - Vital Records website. They have a form for this application. The client will need to gather some proofs - did she take the newborn to the hospital or a doctor shortly after birth - get the certified medical records. Did she use EMS or a private ambulance to get them to the hospital. Was someone else in attendance at the birth who could give an affidavit as to the date, place and time of birth?
