I spoke with Rick at Easy Soft when I saw David's original posting. Rick reminded me that this newest form of CLIS was incorporated more than a year ago in the EasySoft CIS program.

I just found out the hard way...
Attached is the new Confidential Litigant Information form. Complaints that don't include it are being rejected by Case Management. (Also, remember, this form does not get carbon'ed to your adversary - just filed with the court. Common mistake I think a lot of us see on a regular basis).
