    Hi - I rarely do FD work, mostly FN, FG, FM. Client has FD custody of teenager who was in child protection (DCPP) custody. It took less than 6 months to realize the girl is sick, tried to commit suicide, assault on foster father, two admissions into psychiatric hospital, etc. They cannot deal with her any more as they are afraid for safety of their two young kids. I need to do a motion under the FD and notify the Division that they are requesting the teen be removed from their custody and placed in a higher level of care, such as mentor home or supervised partial care home. They are not family of the teen. My question concerns the FD forms that apparently must be used for FD motions. This motion requires a detailed certification and explanation of the issues.There are no check boxes for this. It must be structured like a regular FM or civil motion. The Division's FN case against the bio parents is closed and the Division will not respond. This is Camden -- does anyone have experience trying to get the more complicated FD heard in Camden?

    Use the "click box" form for modification and where it says other relief requested, state what it is you need done and say "see attached certification, Ex. 1"

    Be sure to attach the Confidential Litigant Information Sheet for ALL interested parties or it will immediately be sent back to you. So if there are numerous Plaintiffs or Defendants, CLIS's for all. 

    And... don't forget the dreaded filing fee. (even when I include the fee, I've had them bounced back as they are often "LOST" in Camden)
