Hi - I did this in Gloucester County a couple of yeas ago. The court would permit no "shortcuts." The grandmother had to be fully vetted and investigated etc. by approved social agency (she used Catholic Charities, which still cost in excess of $2000; they were the least expensive). Then we filed Complaint. We got signed and notarized affidavits from the bio parents. That wasn't easy either, since they were homeless on the streets of Camden. The Surrogate's Office helped with the requirements; in fact, I think the list is online on its site. I have the forms I used and if you want, I can e-mail them to you.

I have been retained by paternal grandparents who have under an FD Docket been granted sole and legal custody of their grandchild since 10.2011. They are now seeking to formally adopt the child. Can someone guide me as to what the necessary steps are as they would relate to (1) with consent of the biological parents (dad is definitely on board, not sure about mom) or (2) without consent of one of both parents? Are there specific forms I should use? Any assistance would be greatly appreciated.
